The Complementary Alternative Medicine (CAM) Benefit
Effective Date: 1/1/18 – 12/31/18
Please note: If you are contributing to an HSA, you must meet the plan deductible first.
BeneCAM™ plan is designed to offer all eligible employees and covered dependents a choice when it comes to health care by paying toward the cost of Complementary Alternative Medicine (CAM).
With BeneCAM™ services , such as the following, are covered:
- Acupuncture
- Ayurveda
- Herbal Medicine
- Homeopathy
- Massage Therapy
- Meditation
- Nutrition
- Tai Chi
- Yoga
There is no deductible. Your benefits are as follows:
- The first $250 of CAM therapies will be covered at 100%.
- The next $1,000 will be covered at 50%.
Benefit maximum: $750 per member per year
Members are required to obtain a Letter of Medical Necessity from an MD, DO, NP or PA. After visiting a CAM provider, send the invoice to Tanya Knight. Benemax will pay the benefit to the provider or reimburse you. Be sure that the invoice includes your name, date(s) of service, treatment provided and charges.
