Your Life/AD&D Benefits are insured by Equitable

Effective Date: 8/1/2023 – 7/31/2024

For all Employees who are benefits eligible, Apps Associates provides for Life Insurance up to 1 x the employees salary to a maximum of $200,000. This coverage included AD&D with a matching $200,000. This benefit is paid 100% by Apps Associates.

Voluntary Term Life

Eligibility: All benefits eligible employees and their eligible spouses and children (up to age 19 or to age 23 if they are full-time students fully dependent on the employee for support)

Coverage amounts:

• Employee: up to 5 times salary in increments of $10,000, not to exceed $500,000
• Spouse: up to 100% of employee amount in increments of $5,000
  • Benefits will be paid to the employee
• Child: up to 100% of employee coverage amount of $10,000
  • Benefits will be paid to the employee
